The Columbia space shuttle was destroyed on Feb, 1, 2003, upon reentry due to damage sustained when a section of insulating foam broke loose during launch and damaged several protective tiles on the outer hull of the craft. The damage to these tiles did not allow sufficient protection of the shuttle from heat and stress during reentry, and the shuttle disintegrated.
