These problems include mechanicalfailures such as stuck, broken, or leaking valves, dampers,or actuators; control problems related to failed or drifting sensors;poor feedback loop tuning or incorrect sequencing logic; degradedperformance caused by heat exchanger fouling; design errors; orinappropriate operator intervention.
